We are looking for an experienced Care Professional to join our team at Cygnet Fairways, where you will be responsible for delivering high-quality care and support to individuals with mental health needs.

Your primary duties will include:

  1. Providing individualized care and support to meet the unique needs of each service user;
  2. Developing and implementing individual care plans, working collaboratively with the multidisciplinary team;
  3. Ensuring that all relevant information is up-to-date and accurate, reporting on medical and welfare needs as required;
  4. Supporting individuals to maintain their independence and making informed decisions about their care;
  5. Working in partnership with other healthcare professionals to ensure seamless care delivery.

The ideal candidate will have a strong background in care and a genuine passion for delivering outstanding care. You will be an excellent communicator, a good team player, and possess the energy required to provide a trusting, stimulating, and varied environment.

In return for your hard work and dedication, you can expect a competitive salary of £29,000 - £33,000 per annum, depending on experience, plus a range of benefits, including:
