Job Description:

We are seeking a highly skilled and experienced Senior Practitioner/Social Worker to join our Transitions Service as a Mental Health Team Coordinator. In this role, you will play a key part in identifying those who may need support and overseeing innovative strengths-based assessments with individuals, carers, and their support networks.

Your Key Responsibilities:

  • Advise, support, and supervise a team, encouraging a culture of continued learning and professional development.
  • Ensure the team meets Suffolk County Council's statutory duties under relevant legislation and guidance for adults with support needs and their carers.
  • Enhance team practice to deliver a collaborative response, providing the right information, advice, and enablement to young people to support them in achieving their outcomes in life.
  • Support the development and maintenance of forecasting measures to map future need and help develop services in Suffolk.
  • Interface between children, adult, educational, health, and community services.

Salary and Benefits:

  • Salary: £45,380-£55,634 per annum (pro rata for part-time)
  • 37 hours per week - Flexible working options available
  • Fixed Term or Secondment to 30/06/2025
