Care Coordinator Specialist

We are seeking an experienced Care Coordinator Specialist to join our dedicated team and make a real difference in the lives of individuals with learning disabilities.
Key Responsibilities- Manage complex cases, overseeing challenging caseloads and conducting thorough assessments and reviews.
- Lead and develop the team, sharing expertise and providing supervision to drive best practices.
- Safeguarding champion, ensuring the well-being of individuals through effective management of safeguarding inquiries.
- Collaborate closely with multi-disciplinary teams, including health, education, and housing providers.
- A recognised social work degree and registration with Social Work England.
- Post-qualification certification as a Practice Educator, Best Interest Assessor, or Approved Mental Health Professional.
- Significant experience working with individuals with learning disabilities.
- Strong communication skills, enabling effective interaction with individuals, families, and professionals.
- A genuine passion and commitment to improving the lives of people with learning disabilities.
