Ludlow Street Healthcare is seeking a Care Coordinator to join our team in the Southwest of England and South Wales. As a key member of our staff, you will be responsible for supporting individuals with Autism, Learning Disabilities, Behaviours of concern, Physical Disabilities, Mental Health, Epilepsy and Complex Needs.

About the Role

In this role, you will work closely with our clients to encourage them to participate in their chosen activities or follow their interests. This may involve assisting them in the community, co-ordinating scheduled activities, and promoting independence during personal care tasks. You will also be required to maintain accurate records, including incident forms, daily notes, and finance records.

Requirements
  • Excellent communication and listening skills, as well as good basic comprehension of English language including basic literacy and numerical skills (GCSE level or equivalent).
  • Knowledge/experience/desire to support individuals with Mental Health, Learning Disabilities or Challenging Behaviour.
  • Ongoing commitment to own personal development and training.
  • A valid UK Driving Licence is essential.

The salary for this role is estimated to be around £25,000 - £28,000 per annum, depending on experience. If you are a caring, energetic, enthusiastic individual who is passionate about making a difference in people's lives, we would love to hear from you.
