We are seeking a Personalised Care Advocate to join our team at Advantage Healthcare.
This role involves working closely with clients in their own homes, providing personal care and support to enhance their independence and quality of life.
As a Personalised Care Advocate, you will be responsible for delivering tailored care plans that meet the unique needs and preferences of each client.
This is a rewarding and challenging role that requires strong communication and interpersonal skills, as well as the ability to think creatively and problem-solve.
Key Responsibilities- Develop and implement tailored care plans for each client
- Deliver person-centred care to clients in their own homes
- Collaborate with other healthcare professionals to ensure seamless care delivery
- Degree or equivalent qualification in Health and Social Care
- Minimum 12 months' experience in a care setting
- Exceptional communication and interpersonal skills
- Certification in First Aid and Manual Handling desirable
- A salary of £28,000 - £35,000 per annum
