About the Role

We are seeking a skilled and compassionate Recovery Support Worker – Case Manager to join our team at The Nelson Trust. In this rewarding role, you will provide 1:1 support to clients in our residential addiction treatment setting, helping them achieve long-term change and recovery.

Responsibilities

Key responsibilities include:

  • Conducting regular care reviews to understand clients' individual needs
  • Developing and implementing tailored care plans to support clients' recovery goals
  • Working collaboratively with colleagues to ensure seamless support and services
