Job Title

A rewarding opportunity has arisen for a Dementia Specialist Practitioner to join our client, a leading care provider, at their Specialist Dementia Care Community (SDCC) in Glasgow. This innovative environment is focused on delivering exceptional care for individuals with complex dementia.

The ideal candidate will be passionate about dementia care and possess strong clinical skills to provide comprehensive assessments and develop tailored care plans. As a key member of the multidisciplinary team, you will work collaboratively to deliver high-quality care that meets the unique needs of each resident.

Key Responsibilities:

  • To conduct comprehensive assessments of residents with dementia, identifying their needs and developing individualized care plans.
  • To work collaboratively with the multidisciplinary team to deliver person-centered care that meets the physical, emotional, and social needs of residents.
  • To provide education and support to colleagues and family members on dementia care best practices.
  • To stay up-to-date with the latest research, guidelines, and evidence-based practices in dementia care.

What We Offer:
