SEND Practitioner

The Education Health and Care Team at Bath & North East Somerset Council is seeking a talented individual to join their team as a SEND Practitioner. This role is focused on person-centred planning, co-production, and outcomes to generate outstanding and effective Education Health and Care Plans to support children and young people with their education and preparing for adulthood.
The successful candidate will be responsible for coordinating the collection of assessments required to support SEND processes, networking with various services and agencies to gather information, and attending EHCP Annual Review meetings. Excellent interpersonal skills, the ability to independently plan workload, and strong written and verbal communication skills are essential for this role.
Key Responsibilities

About You
The ideal candidate will be energized by working under pressure, able to switch between competing priorities, and comfortable using their initiative. They will also have excellent analytical and interpretive skills, as well as the ability to summarise information concisely.
