SEND Practitioner

The Education Health and Care Team is seeking a talented SEND Practitioner to join their team. As a key member of the team, you will be responsible for person-centred planning, co-production, and outcomes to generate outstanding and effective Education Health and Care Plans.
You will work closely with relevant professionals throughout statutory SEND processes, ensuring that children and young people receive the best possible support. Your duties may include:
- Coordinating the collection of assessments required to support SEND processes for new requests for assessment and Annual Reviews.
- Networking with various services, agencies, and providers to gather information and support families in making informed choices.
- Attending EHCP Annual Review meetings and ensuring reviews are outcome-focused, person-centred, and completed within statutory timeframes.
About You
To be successful in this role, you will need excellent interpersonal skills, the ability to independently plan your workload, and a passion for working under pressure. You will also need to be comfortable using your initiative, analysing and interpreting information from a wide range of sources, and possessing excellent written and verbal communication skills.
