About the Role

The Education Health and Care Team at Bath & North East Somerset Council is seeking a talented SEND Practitioner to join their team. As a SEND Practitioner, you will play a crucial role in supporting children, young people, and their families throughout statutory SEND processes.

Key responsibilities include coordinating the collection of assessments, producing high-quality Education Health and Care Plans, and networking with various services and agencies to gather information and support families in making informed choices.
