Job Summary
We are seeking an exceptional Advanced Nurse Practitioner to join our VAWAS TEAM (Vulnerable Adults Wrap Around Service) at Prospect Health. The successful candidate will be an exceptional team player with good communication and interpersonal skills who is enthusiastic and committed to delivering high-quality patient-focused care to our most vulnerable patients.
About the Role
The ideal candidate will have a sound background in chronic disease management, first contact and telephone triage, assessing, treating, and reviewing all ages and conditions. They must have at least two years' experience as an Advanced Nurse Practitioner and hold a non-medical prescribing qualification.
Key Responsibilities
- Assess, diagnose, plan, implement, and evaluate treatment/interventions and care for registered patients presenting with an undifferentiated diagnosis and long-term chronic conditions.
- Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
- Assess, diagnose, plan, implement, and evaluate interventions/treatments for patients with complex needs.
- Deliver LTC reviews and venepuncture when required.
- Complete DNACPR forms.
- Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
- Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
Requirements
- Relevant nursing/health MSc/degree in Advanced Nursing Practice.
- Minimum 3 years post-registration experience.
- At least 2 years recent primary and/or community nursing experience.
- Nurse-led management of minor illness, minor ailments.
- Nurse-led triage experience.
