
Clinical Lead Nurse Practitioner

We are seeking an experienced and skilled Advanced Nurse Practitioner to join our VAWAS team, providing high-quality care to vulnerable adults.
About the Role:
- Deliver comprehensive assessment, diagnosis, and treatment plans for patients with complex needs.
- Clinically examine and assess patient needs from a physiological and psychological perspective.
- Proactively identify, diagnose, and manage treatment plans for patients at risk of developing long-term conditions.
Key Responsibilities:
- Assess, diagnose, plan, implement, and evaluate treatment/interventions and care for registered patients presenting with undifferentiated diagnoses and long-term chronic conditions.
- Deliver LTC reviews and venepuncture when required.
- Complete DNACPR forms.
Requirements:
- A relevant nursing/health MSc/Degree in Advanced Nursing Practice.
- Minimum 3 years post-registration experience.
- At least 2 years recent primary and/or community nursing experience.
- Nurse-led management of minor illness and minor ailments.
- Nurse-led triage experience.
