We are seeking a highly motivated and experienced Registered Mental Health Nurse to join our Community Mental Health Team as a Care Coordinator. This is an exciting opportunity to work in a dynamic and innovative environment, providing high-quality care to individuals with severe mental health problems.
Key Responsibilities- Assess, implement, and evaluate care and treatment plans for individuals on the caseload.
- Work autonomously as part of a multidisciplinary team, under the indirect supervision of a Band 7 registered nurse/team manager.
- Collaborate with unregistered healthcare assistants, other health colleagues, and local authority colleagues to deliver a supportive service.
- Participate in mental health assessments and provide a point of contact for existing service users in the absence of their care coordinator.
- Support clinic nurses during times of leave, including depot, Clozapine, Lithium, and physical health screening clinics.
- Registered Mental Health Nurse qualification.
- Postgraduate qualification (diploma, degree, or higher qualification) or evidence of working towards the same.
- Experience of working at Band 5 in a multidisciplinary team framework in Acute Adult Mental Health Services.
- Experience of joint working with other agencies/services.
- Ability to work autonomously, communicate effectively with service users, and travel between sites.
- Knowledge of evidence-based practices, health board policies, medication, risk assessment, and clinical audit/governance.
