About the Role:

We are seeking a highly skilled Registered Mental Health Nurse/Practitioner to join our dynamic Community Mental Health Team for Older People in South Somerset. Based in Chard or Yeovil, this rewarding opportunity will allow you to make a positive impact on the lives of individuals and their families.

Job Summary:

You will be responsible for managing a caseload of people with mental health difficulties, providing assessment and evidence-based interventions. Your expertise will be essential in supporting clients with complex needs, working closely with their significant others to deliver vital support and interventions.

Main Responsibilities:

  • Provide psychosocial interventions to clients with mental health difficulties
  • Work collaboratively with multi-disciplinary teams to deliver comprehensive care
  • Develop and implement individualized care plans to meet client needs

Requirements:

  • Current registration with relevant professional body (e.g., NMC)
  • Relevant degree or equivalent experience
  • Experience of working with families, carers, and significant others is desirable
