Job Title: Support Worker

Job Summary:

We are seeking a dedicated and compassionate Support Worker to join our team at Tezlom, a leading agency in Health and Social Care. As a Support Worker, you will play a vital role in providing care and support to young adults with mental health needs and learning disabilities in Harrow, Northolt, and Pinner.

Key Responsibilities:

  • Provide individualized support to young adults with complex mental health needs and learning disabilities.
  • Work collaboratively with a multidisciplinary team to develop and implement personalized care plans.
  • Support young adults with daily living tasks, including personal care, meal preparation, and household chores.
  • Encourage and facilitate social interaction and community engagement.
  • Monitor and report any changes in the young adults' behavior or well-being.

Requirements:

  • At least 2 years of experience working with mental health needs and learning disabilities in a residential or care home setting.
  • Ability to work as part of a team and provide support to colleagues as needed.
  • Commitment to working with young adults who may exhibit challenging behavior.
  • Flexibility to work a variety of shifts, including evenings and weekends.
