**Job Summary**

We are seeking a part-time Support Worker to join our Whānau Support Services team in the Nelson area. As a Support Worker, you will play a vital role in supporting individuals with disabilities to live independently, achieve their goals, and participate in their communities.

**Key Responsibilities**

  • Provide personal care and support to individuals with disabilities, including assistance with daily living tasks, budgeting, and meal preparation.
  • Assist individuals with disabilities to participate in community activities, such as outings, events, and social gatherings.
  • Support individuals with disabilities to achieve their goals and aspirations, including education, employment, and personal development.
  • Work collaboratively with other healthcare professionals, family members, and caregivers to ensure comprehensive support for individuals with disabilities.
  • Maintain accurate records and reports, as required.

**Requirements**

  • Previous experience in a healthcare setting, preferably with individuals with disabilities.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to maintain confidentiality and handle sensitive information.
  • Full driver's license and access to a reliable vehicle.
