Female Local Service Manager

The Service Coordinator plays a vital role in ensuring the smooth intake process and coordination of services for families. This position requires strong communication and interpersonal skills to effectively interact with clients, colleagues, and community partners.
Key Responsibilities:- Conduct initial telephone interviews or arrange for in-person meetings to gather essential information for admission decisions.
- Participate in Multi-Disciplinary Team meetings to review cases and develop plans for client support.
- Complete intake forms, input required information into the client database, and maintain accurate records in accordance with established standards.
- Stay informed about Merrymount services and programs to provide effective support to clients.
- Coordinate services within Merrymount and refer clients to relevant community agencies as needed.
- Bachelor of Social Work degree with current registration with the College of Social Workers.
- Minimum 2 years' experience in interviewing, crisis intervention, and related human services interventions, preferably involving client needs assessment and case management.
- Training and experience in using assessment tools and screeners.
- Excellent verbal and written communication skills, including effective telephone communication and interviewing skills.
- Knowledge of computer systems and databases.
- Bilingual in English and French is considered an asset.
