We are seeking a talented and dedicated Coordinated Healthcare Professional to join our team at The Human Support Group Limited. In this role, you will have the opportunity to make a real impact on the lives of our clients by providing exceptional care coordination.

Salary: £30,000 - £40,000 per annum, depending on experience.

Your responsibilities will include coordinating care plans, managing rota's, and building relationships with care workers. We are looking for someone who is efficient, organised, and passionate about delivering high-quality care.

We offer a comprehensive benefits package, including up to 14% discount at over 40 retailers, wellbeing resources, financial advice, and occupational sick pay. Additionally, you will receive 22 days holiday (increasing to 25 days after 3 year's service) plus bank holidays, occupational maternity pay & adoption pay, occupational paternity pay, death in service payment, and much more.
