Job Role Overview



The role of the Field Care Supervisor at City and County Healthcare Group involves supporting the delivery of high-quality care support services. This includes carrying out various functions in the field, such as assessing and reviewing individual services, and providing practical supervision, instruction, and support to care workers.



Key Responsibilities




  • Supporting the planning of realistic, effective care worker rotas in collaboration with care coordinators, taking into account the individual needs of the client and the skills and attributes of the care worker.
  • Carrying out regular reviews of individual services, including repeated and renewed needs and risk assessments.
  • Participating in multi-disciplinary reviews of service users' needs as required.
  • Shadowing new care workers in the field to ensure their competence.
  • Conducting on-site observations and evaluations of care worker practice as part of routine supervision or in response to specific performance concerns.

Requirements



To be successful in this role, you will need experience in carrying out initial risk and needs assessments for individual service users, either in their homes or in other settings. You will also need to work in partnership with service users, their families, carers, advocates, healthcare professionals, and others to ensure the seamless delivery of holistic packages of care support.
