About the Role

This is a full-time permanent position, working closely with clinical pharmacists, community pharmacists, and other members of the Primary Care Network (PCN) multi-disciplinary team. The ideal candidate will have excellent communication and organisational skills, as well as experience in pharmacy practice.

Key responsibilities include:

  • Undertaking medicines reconciliation in accordance with practice protocols and updating patients' medical records accordingly.
  • Providing advice to GPs, staff, and patients on changes to prescribing, ensuring safety, quality, and cost-effectiveness.
  • Liaising with the medicines management team to monitor and manage prescribing budgets.
