Bradford District Care NHS Foundation Trust is committed to providing high-quality care for the people of Bradford, Airedale, and Craven. We are seeking a professionally qualified RMN with experience in care coordination to join our vibrant specialist psychosis service.

The successful candidate will be responsible for assessing, planning, and providing packages of care for individuals and their families/carers experiencing first episode psychosis. This role requires working in partnership with service users, carers, and other partner agencies to fulfill all requirements of CPA Care Co-ordination.

Key qualifications include a professional qualification in mental health nursing (RMN/RLDN), knowledge of the principles of EIP, and experience of care coordination in a mental health service. Desirable skills include degree-level education, community language skills, and experience working with adults and/or young people and children with serious mental health problems.
