Job Title: Care Pathway Coordinator

We are seeking a dedicated and compassionate Care Pathway Coordinator to support young people in achieving their full potential.

Key Responsibilities:

  1. Assess and monitor the needs of young people, ensuring plans remain effective and tailored to each individual.
  2. Provide personal and emotional support, including assisting with domestic tasks and providing transport when necessary.
  3. Advise young people on important aspects of life after care, such as health, benefits, education, training, housing, employment, and personal well-being.
  4. Help young people make informed decisions, self-advocate, and engage fully in the development of their Pathway Plans and Personal Action Plans.
  5. Support young people as they transition from childhood to adulthood, empowering them to take responsibility for their future.

Essential Requirements:

  • NVQ Level 3 (or equivalent) in a relevant field.
  • Experience working directly with young people, particularly those in care or recently leaving care.
  • Ability to work creatively with young people, including those with complex needs or in vulnerable situations.
  • Understanding of the challenges young people face regarding housing, education, training, and employment.
  • Experience collaborating with other professionals and agencies to support young people.
  • Full UK driving licence and access to a car or reliable transport for work purposes.

Contract Details:

This is a full-time position based in Bradford, with a 3-month initial contract (with potential for extension). The salary range is £25,000 - £30,000 per annum, depending on experience.
