Job Description
We are seeking an experienced Registered Mental Health Nurse or Learning Disability Nurse to join our team as a Mental Health Services Coordinator. In this role, you will be responsible for coordinating care and support services for patients with complex mental health needs.

Your key responsibilities will include:
  • Coordinating patient assessments and care planning.
  • Collaborating with multidisciplinary teams to ensure comprehensive patient care.
  • Developing and implementing individualized care plans that address the unique needs of each patient.
