Job Title: Band 6 Outreach Nurse Specialist

We are seeking an experienced and skilled Band 6 Outreach Nurse Specialist to join our team at University Hospitals of Morecambe Bay NHS Foundation Trust. As a key member of our Multidisciplinary team, you will play a vital role in delivering high-quality care to acutely unwell patients in the community.

Main Responsibilities:
  • Support the development of our Virtual Ward and 2-hour urgent community response standard, providing crisis response to patients who are acutely unwell within our local community.
  • Work collaboratively with our team to deliver care to patients in their homes, community centres, clinics, and GP Practices.
  • Assess and support learners in practice, ensuring they receive high-quality training and development opportunities.
  • Prescribe medication and provide evidence-based care to patients with acute and chronic health conditions.
Person Specification:
  • Registered Adult Nurse with extensive post-registration community or hospital experience.
  • Broad understanding of acute and chronic health conditions.
  • Prescribing Qualification and evidence of post-registration clinical and professional qualifications/continuous development in related health field.
