Join Our Community Care Team

We are seeking a highly skilled Band 6 nurse to join our Furness Rapid Response Team at University Hospitals of Morecambe Bay NHS Trust.

As a key member of our multidisciplinary team, you will play a vital role in delivering care to acutely unwell patients in the community, working closely with Advanced Clinical Practitioners, Nurses, Occupational Therapists, Physiotherapists, and Support Workers.

Main Responsibilities
  • Support the development of our Virtual Ward and 2-hour urgent community response standard, providing crisis response to patients who are acutely unwell within our local community.
  • Work collaboratively with the team to deliver high-quality care, ensuring the best possible outcomes for patients.
  • Contribute to the ongoing development of our community care services, identifying opportunities for improvement and implementing changes as needed.
Requirements
  • Registered Adult Nurse with extensive post-registration community or hospital experience.
  • Broad understanding of acute and chronic health conditions, with a focus on community-based care.
  • Audit experience and a Prescribing Qualification are highly desirable.

  • Demonstrate a commitment to ongoing professional development and continuous learning.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Able to work independently and as part of a team, with a flexible approach to working hours and shifts.
