Join Our Community Care Team

We are seeking a highly skilled and experienced Band 6 nurse to join our Furness Rapid Response Team at University Hospitals of Morecambe Bay NHS Trust.

As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality care to acutely unwell patients in the community, working closely with Advanced Clinical Practitioners, Nurses, Occupational Therapists, Physiotherapists, and Support Workers.

Main Responsibilities
  • Support the development of our Virtual Ward and 2-hour urgent community response standard, providing crisis response to patients who are acutely unwell within our local community.
  • Contribute to the ongoing development of our community care services, working collaboratively with colleagues to ensure seamless care delivery.
  • Assess and support patients in their own homes, providing expert care and guidance to help them manage their conditions.
  • Work closely with our multidisciplinary team to ensure effective communication and coordination of care.
Requirements
  • Registered Adult Nurse with extensive post-registration community or hospital experience.
  • Broad understanding of acute and chronic health conditions.
  • Audit experience and a Prescribing Qualification.
