Job Title: Discharge Lounge Care Coordinator

Job Summary: We are seeking a skilled Discharge Lounge Care Coordinator to join our Patient Flow Support Team at NHS Scotland. The ideal candidate will have excellent communication skills, be able to work autonomously, and possess strong organizational abilities.

Key Responsibilities:

  1. Provide exceptional care to patients in the Discharge Lounge, ensuring a smooth transition to their next stage of care.
  2. Work closely with the Patient Flow Support Team to identify areas of improvement and implement changes to optimize patient flow.
  3. Develop and maintain strong relationships with patients, families, and healthcare professionals to ensure seamless communication and care.

Requirements:

  • Current registration as a registered nurse with the relevant professional body.
  • Proven experience in patient flow management, acute hospital settings, or similar environments.
  • Excellent communication and interpersonal skills.
