We are seeking a highly skilled and dedicated individual to join our team as an Advanced Community Healthcare Practitioner. The successful candidate will be responsible for providing high-quality healthcare services to individuals in the community, with a focus on severe and enduring mental health needs.

About the Role

The ideal candidate will have a professional qualification in Mental Health Nursing, Social Work or Occupational Therapy, with evidence of personal and professional development post-qualification. They will also have experience in risk assessing, planning, and implementation, as well as good organisational skills and the ability to work autonomously.

A strong understanding of the principles of care coordination and a commitment to person-centred care are essential. The successful candidate will be required to maintain accurate records and provide reports within agreed timeframes.

Key Responsibilities
  • Conduct comprehensive mental health assessments for service users with severe and enduring mental health needs.
  • Develop and deliver treatment programmes using evidence-based practice.

Salary and Benefits

We offer a competitive salary of £35,000 - £45,000 per annum, depending on experience, plus excellent benefits including a pension scheme, life insurance, and access to training and development opportunities.

The estimated salary range is based on the National Health Service (NHS) pay scales for a Band 6 post, taking into account the location and the level of responsibility involved.
