We are seeking a dedicated and compassionate Community Health Practitioner to join our team in Bristol.

**Key Responsibilities:**

- Providing person-centred care to patients in their own homes
- Developing and implementing individualised care plans with patients, families, and carers
- Collaborating with healthcare professionals to ensure seamless care coordination

**What We Offer:**

- Salary: £32,000 - £36,000 per annum (dependent on experience)

**Requirements:**

- 1+ years of experience as a registered nurse (RGN/RMN/RPN) or equivalent qualification
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Excellent time management and organisational skills

**Estimated Salary Range:** £35,400 - £39,600 per annum
