Band 6 Community Nurse Out Of Hours Specialist

We are seeking a highly skilled and dedicated Band 6 Community Nurse to join our North Kent Community Nursing Out of Hours Service, based at Sheppey Community Hospital. As a key member of our team, you will play a vital role in delivering high-quality patient-centered care to individuals and their families in the community.
Key Responsibilities- Provide holistic care to patients and their families, promoting independence and well-being.
- Work collaboratively with the multidisciplinary team to ensure seamless care delivery.
- Develop and maintain strong relationships with patients, families, and community stakeholders.
- Participate in regular team meetings and contribute to the development of best practices.
- Engage in ongoing professional development to enhance clinical skills and knowledge.
