About the Role

We are seeking a highly skilled Band 6 Community Nurse to join our Out of Hours Service at Sheppey Community Hospital. As a key member of our team, you will provide high-quality patient care and support to patients and their families in the North Kent area.

Key Responsibilities

  • Deliver holistic patient-centered care to patients and their families in the community
  • Work collaboratively with the multidisciplinary team to provide comprehensive care
  • Develop and implement individualized care plans to meet the unique needs of patients
  • Provide education and support to patients and their families to promote health and well-being
