At Service Care Solutions Ltd, we are seeking a highly skilled and experienced Senior Family Support Worker to join our team. This is a challenging and rewarding role that requires strong management skills, attention to detail, and a passion for supporting families with children with disabilities.

About the Role

We are looking for an individual who can manage the Children with Disabilities Project in Redbridge, ensuring that all young people receive the support they need to thrive. Key responsibilities include undertaking and monitoring reviews of children with disabilities, identifying child protection issues, and developing a network of links between Health, Education, and Social Services.

Key Responsibilities
  • Undertake and monitor reviews of children with disabilities to minimize their disability and facilitate them staying within their own family unit.
  • Identify child protection issues and refer them to Senior Practitioners or Team Manager.
  • Complete Transition Assessments, Mental Capacity Assessments, and Care and Support Plans for young people.
  • Develop a network of links between Health, Education, and Social Services.
  • Complete Continuing Health Care Checklists for young people.
Requirements

To be successful in this role, you will need:

  • Fully enhanced DBS
  • Experience of parenting courses through group or one-to-one sessions
