Community Staff Nurse

About the Role
We are seeking an experienced and motivated nurse to join our Community Nursing Team as a Community Staff Nurse. This role will provide clinical care and support to patients in their own homes and residential settings.
Main Responsibilities
The successful candidate will be responsible for providing high-quality, evidence-based care to patients with complex and often high-acuity needs. This will involve working closely with GP practices and other partners to prevent unnecessary hospital admissions.
Requirements
- Registered as fit to practice by the Nursing and Midwifery Council (NMC)
- Experience of multi-disciplinary working
- Commitment to the needs of the service, including a flexible and adaptable approach
