Community Staff Nurse Position

We are seeking a highly skilled and motivated Community Staff Nurse to join our team at South Tees Hospitals NHS Foundation Trust. As a Community Staff Nurse, you will play a vital role in providing high-quality, patient-centered care to individuals in their own homes and residential settings.
Main Responsibilities- Provide clinical care and support to patients with complex and often high-acuity needs.
- Work closely with GP practices and other partners to prevent unnecessary hospital admissions.
- Develop and implement effective care plans to meet the needs of patients with diverse healthcare and social needs.
- Collaborate with multidisciplinary teams to ensure seamless care delivery.
- Participate in quality improvement initiatives to enhance patient outcomes and experiences.

Requirements- Registered as fit to practice by the Nursing and Midwifery Council (NMC).
