Job Summary

We are seeking a highly motivated and experienced Community Staff Nurse to join our team at South Tees Hospitals NHS Foundation Trust. As a Community Staff Nurse, you will provide high-quality, patient-centered care to individuals in their own homes and residential settings.

Main Responsibilities

The successful candidate will be responsible for:

  • Providing clinical care and support to patients with complex and high-acuity needs
  • Working closely with GP practices and other partners to prevent unnecessary hospital admissions
  • Developing and implementing innovative practice and supporting the District Nurse Sister and Clinical Lead
  • Collaborating with multidisciplinary teams to deliver effective, evidence-based care

Requirements

To be successful in this role, you will need:

  • Registration as a fit-to-practice nurse with the Nursing and Midwifery Council
  • Experience of multidisciplinary working and a commitment to team objectives
  • Good communication and interpersonal skills
  • Ability to use initiative and make decisions within your sphere of work
