Job Summary

We are seeking an experienced and motivated Community Staff Nurse to join our team at South Tees Hospitals NHS Foundation Trust. As a key member of our Community Nursing Team, you will provide high-quality, patient-centred care to individuals in their own homes and residential settings.

Main Responsibilities

The successful candidate will be responsible for:

  • Providing clinical care and support to patients with complex and high-acuity needs
  • Working closely with GP practices and other partners to prevent unnecessary hospital admissions
  • Developing and implementing innovative practice to improve patient outcomes
  • Supporting the District Nurse Sister and Clinical Lead in the development of our teams
