Main ResponsibilitiesThe successful candidate will be responsible for:
- Providing clinical care and support to patients with complex and high-acuity needs
- Working closely with GP practices and other partners to prevent unnecessary hospital admissions
- Developing and implementing innovative practice to improve patient outcomes
- Supporting the District Nurse Sister and Clinical Lead in the development of our teams
