About the Role

We are seeking a highly skilled and motivated Practice Nurse Specialist to join our team at Riverside Medical Practice.

The ideal candidate will have a strong background in primary care and be able to provide high-quality nursing care to our patient population.

Key Responsibilities
  • Deliver high-quality practice nursing care to patients with long-term conditions, including hypertension, diabetes, asthma, and COPD.
  • Provide cytology services, including smear tests and results.
  • Administer childhood vaccinations and provide guidance on vaccination schedules.
  • Conduct swabs, take blood samples, and measure blood pressures.
  • Maintain accurate and detailed health records on our clinical system.
  • Contribute to Quality and Outcomes Framework (QOF) achievement.
  • Participate in multidisciplinary team (MDT) meetings and discussions.
  • Provide telephone consultations and advice to patients.
  • Discuss blood results with patients and provide guidance on next steps.
Additional Responsibilities
  • Support the wider team with clinical audits, safeguarding concerns, and shared learning initiatives.
  • Assist GPs with patient care and provide support with clinical practices.
  • Review and respond to national policies and initiatives.
  • Participate in local initiatives to enhance patient care.
  • Train and support other members of the MDT.
  • Help organize and run vaccine clinics as needed.
Person Specification

Essential Qualifications

  • NMC Registered Nurse.
  • Qualified in Cytology.
  • Qualified in childhood vaccinations.

Desirable Qualifications

  • Qualified in Phlebotomy.
  • Qualified in Chronic Disease Management.
  • Qualified in wound management.
