Key Responsibilities- Provide care in long-term condition management, including hypertension, diabetes, asthma, and COPD
- Conduct cytology and child immunizations
- Administer vaccinations and perform swabs, specimens, bloods, and blood pressures
- Keep accurate and detailed health records on our clinical system
- Contribute to Quality and Outcomes Framework (QOF) achievement
- Participate in multidisciplinary team (MDT) meetings and discussions
- Provide telephone consultations and advice to patients
- Discuss blood results with patients

Person Specification- NMC Registered Nurse
- Qualified in cytology and childhood vaccinations
- Desirable qualifications in phlebotomy, chronic disease management, and wound management
- Team player with excellent communication and interpersonal skills
- Flexible and adaptable with a focus on providing quality care
- Ability to work in a changing environment and take initiative
- Experience of working in general practice and dealing with vulnerable patients
