Care Manager Job Description

We are seeking a highly skilled and compassionate nurse to join our team as a Clinical Team Leader in palliative care. This is an excellent opportunity for a qualified paediatric or dual registered nurse with a passion for delivering high-quality patient care to make a real difference in the lives of children and young people with life-limiting conditions.

About the Role:

In this dynamic and supportive role, you will provide leadership and mentorship to the care team, manage the needs of the children and young people, and develop and implement care plans that meet their individual needs. As part of the senior clinical team, you will ensure day-to-day review of skill mix and care needs to maintain high standards of care.

Key Responsibilities:

• Provide clinical leadership and support to the care team
• Develop and implement care plans that meet the individual needs of each child or young person
• Manage the care team to ensure high standards of care are maintained
• Collaborate with other teams to develop services across both hospices and the community

Requirements:

• Qualified paediatric or dual registered nurse with proven clinical leadership skills
• Up-to-date competence in invasive and non-invasive respiratory support, IV and central line management, and end-of-life care
• Excellent interpersonal skills
• Driving licence essential due to rural location

Estimated Salary: £62,500 per annum
