We are seeking an experienced Clinical Discharge Navigator Lead Physiotherapist to join our team at the Royal London Hospital. This is a challenging yet rewarding role that requires strong leadership skills, clinical expertise, and a passion for improving patient care.


Job Description


The successful candidate will be responsible for leading the discharge assessment and planning process for patients with complex needs, working closely with multidisciplinary teams to ensure timely and safe discharges. You will also coordinate early complex discharges, identify potential delays, and lead on problem-solving/case review meetings.


In this role, you will have the opportunity to promote safe and effective discharge practice throughout the trust, act as a resource for expert teaching and guidance, and supervise and line-manage more junior staff.


Requirements


To be successful in this role, you will need:



  • A relevant degree/diploma and registration as a senior therapist
  • Experience of working in an acute/tertiary hospital setting, including discharge planning and liaison work
  • Strong communication and leadership skills, with the ability to work effectively within a multidisciplinary team
  • Able to demonstrate clinical experience/expertise and a strong understanding of discharge planning processes, legislation, and policies

About the Role
