Job Title: Social Worker for Older Adults

We are seeking a skilled Social Worker to join our team in Conwy County, working with older adults and their families to provide high-quality social work services.

About the Role:

This is a short-term assignment for 37 hours per week, Monday to Friday, with the possibility of extension. The successful candidate will work from our office in Llys Dyfrig, Llandudno, Conwy, and will be expected to travel throughout the county and beyond to facilitate hospital discharges.

Key Responsibilities:
  • Deliver a high-quality social work service to older adults and their families
  • Work collaboratively with citizens, carers, and families to provide support and develop resilience and independence
  • Ensure that the needs and preferences of older adults are at the centre of decision-making
  • Develop and implement effective care plans to meet the needs of older adults
Requirements:
  • Social Work qualification and registration with Social Care Wales or Social Work England
  • Minimum 3 years post-qualified experience
  • Car driver with access to a car
  • Excellent communication and interpersonal skills
  • Enhanced DBS registration or willingness to apply
