Care Coordinator

About the Role
We are seeking a highly skilled and dedicated Care Coordinator to join our team at Greater Middlesbrough Primary Care Network (PCN). As a key member of our multidisciplinary team, you will play a vital role in supporting the delivery of high-quality patient care.
Key Responsibilities
- Prioritize workload effectively and manage competing demands in accordance with Practice, local, and national guidelines.
- Contribute to the administration of the Practice, including general administrative duties and QOF management.
- Develop and maintain a strong understanding of clinical systems, including SystemOne, and apply this knowledge to support the team.
- Collaborate with colleagues to identify and implement process improvements, ensuring seamless workflow and efficient use of resources.
- Provide exceptional patient care by responding to patient needs, addressing concerns, and promoting a positive patient experience.
- Support the development and implementation of new IT projects, ensuring effective integration with existing systems.
- Manage staff training requirements, maintain accurate records, and ensure compliance with regulatory requirements.
- Contribute to the maintenance of accurate and up-to-date records, including patient information and Practice documentation.
- Participate in regular team meetings, contribute to discussions, and provide input on key issues and initiatives.
Requirements
- High standard of IT skills, including Microsoft Office Applications and clinical systems.
- Previous NHS experience, with a strong understanding of Medical Terminology and ability to work on own initiative.
- Excellent interpersonal skills, with the ability to work effectively as part of a team.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Flexibility and adaptability to meet the evolving needs of General Practice.
