Care Coordinator

About the Role
We are seeking a highly skilled and dedicated Care Coordinator to join our team at Greater Middlesbrough Primary Care Network (PCN). As a key member of our multidisciplinary team, you will play a vital role in supporting the delivery of high-quality patient care.
Key Responsibilities
- Prioritize workload and manage administrative tasks in accordance with Practice, local, and national guidelines.
- Manage QOF targets and recall in line with patient care requirements.
- Assist with IT problem-solving and lead on the implementation of new IT projects.
- Organize and attend meetings, including GSF, and maintain accurate records.
- Process Subject Access Requests and Insurance reports.
- Develop and maintain strong relationships with patients, carers, and colleagues.
Requirements
- High standard of IT skills, including Microsoft Office Applications and clinical systems.
- Previous NHS experience and good understanding of Medical Terminology.
- Ability to work on own initiative, recognize confidentiality, and handle sensitive information.
- Good interpersonal skills and ability to work effectively as part of the Practice team.
