About the Role

We're seeking a compassionate and dedicated Care Coordination Specialist to join our team at Connected Health.

As a Care Coordination Specialist, you will play a vital role in ensuring the highest standards of care for our clients. Your primary responsibility will be to provide administrative support to our care teams, ensuring that all client needs are met in a timely and efficient manner.

Key Responsibilities
  • Monitor client satisfaction and staff satisfaction online
  • Create client filing systems, both paper and digital
  • Process and file relevant paperwork
  • Archive ceased client paperwork
  • Create client information files for new clients
  • Audit client daily record sheets
  • Assist with answering incoming phone calls from clients, carers, social workers, and Trust employees
Requirements
  • A minimum of 1 year's administration experience within the healthcare sector
  • Proven track record in a fast-paced environment
  • Strong desire to contribute to the overall success of the care coordination team
  • Proficient use of Microsoft Office, including Excel and Outlook
  • Excellent organisational and communication skills
  • Previous experience providing high-level customer service
