Care Coordinator for Adults with Learning Disabilities

About the Role:
We are seeking a highly motivated and compassionate Registered Nurse (RGN) to join our team as a Care Coordinator for adults with learning disabilities at our Shaftsbury Mews nursing service in Whiteabbey. As a Registered Nurse, you will play a vital role in providing exceptional care to our service users, promoting their independence, dignity, and well-being.
Responsibilities:
- To assess and meet the complex needs of individuals with learning disabilities or acquired brain injuries, ensuring their safety and comfort in a supportive environment
- To develop and implement personalized care plans that cater to individual needs and goals, collaborating with multidisciplinary teams to ensure seamless care delivery
- To maintain accurate records and report any changes or concerns to the appropriate authorities
- To engage in continuous professional development to stay updated on best practices and evidence-based approaches in adult care
Requirements:
- Degree in Nursing or equivalent qualification
- Current registration with the NMC
- Proven experience working with adults with learning disabilities or acquired brain injuries
- Excellent communication and interpersonal skills
- Able to work flexibly and efficiently in a fast-paced environment
