Develop and implement innovative strategies to assist individuals who frequently utilize various unplanned services without receiving adequate support. Engage directly with adults in their homes or communities, addressing non-medical needs that cannot be resolved through regular visits to Emergency Departments. Initiate personalized care discussions, starting with a 'What Matters to You' conversation to ensure that care aligns with individual priorities and requirements. Collaborate with various stakeholders to integrate additional support services as necessary.Candidate Profile:
A solid understanding of health disparities and the broader factors influencing health outcomes. Proven experience as a case-holding practitioner or worker, with a commitment to personalized care approaches. A coaching qualification or training in motivational interviewing, appreciative inquiry, or similar methodologies.
