We are seeking a skilled and dedicated Primary Care Nurse to join our team at Maple Surgery in Bar Hill, Cambridge. This is an exciting opportunity for a professional with a strong understanding of quality assurance and a passion for delivering high-quality care to patients.
About the RoleThis full-time position offers the chance to work collaboratively with a supportive and stable clinical team, including 2 GPs, Nurse Practitioners, and an HCA, as well as a Social Prescriber, Community Matron, and a Pharmacy Technician. You will be responsible for ensuring the delivery of safe and effective nursing care to the practice population and accountable for nursing service delivery.
The ideal candidate will have experience working in a primary care environment and possess excellent communication skills. They will also have a commitment to ongoing professional development and be able to follow clinical policy and procedure.
Responsibilities- Support patients in making lifestyle modifications to improve their health outcomes and prevent disease progression.
- Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects to patients' health.
- Coordinate care for patients with complex health needs, liaising with other healthcare professionals and community services as necessary.
- Collaborate with multidisciplinary teams to develop and implement care plans that address the holistic needs of patients.
Estimated salary: £40,000 - £55,000 per annum depending on experience.
