Community Health Specialist

We are seeking a highly skilled and dedicated Community Nurse to join our team at the Royal Devon University Healthcare NHS Foundation Trust. As a key member of our Community Nursing team, you will play a vital role in delivering high-quality patient care to our housebound patients.
Key Responsibilities- Deliver comprehensive nursing care to patients in their own homes, focusing on prevention, early intervention, and management of chronic conditions.
- Work collaboratively with multidisciplinary teams, including GPs, social workers, and other healthcare professionals, to ensure seamless care coordination.
- Conduct thorough assessments, develop care plans, and implement evidence-based interventions to improve patient outcomes.
- Provide education and support to patients and their families, promoting self-management and empowerment.
- Participate in quality improvement initiatives, contributing to the development of best practices and policies within the team.
To be successful in this role, you will need:
- Registration with the Nursing and Midwifery Council (NMC) as a qualified nurse.
- Experience in community nursing, preferably in a similar setting.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Ability to work autonomously, prioritizing tasks and managing time efficiently.
- Commitment to ongoing professional development, staying up-to-date with best practices and research in community nursing.
