About the Role

We are seeking a highly skilled and experienced Physical Health Nurse to join our team at Elysium Healthcare. As a Physical Health Nurse, you will play a vital role in leading and coordinating the physical health needs of our service users, while also supporting your colleagues and promoting good teamwork with the multidisciplinary team.

Key Responsibilities
  • Monitor and assess the physical health needs of service users, identifying areas for improvement and developing strategies to address these.
  • Develop and implement physical health interventions, using knowledge of current best evidence-based practice.
  • Work collaboratively with primary health, developing and maintaining links with GP practices and other physical health departments.
  • Monitor and maintain Physical Health Dashboards, ensuring that data is accurate and up-to-date.
  • Provide education and training to colleagues on physical health topics, promoting evidence-based practice.
Requirements
  • A relevant nursing qualification and registration.
  • Phlebotomy skills.
  • Strong team-working skills.
  • A high level of self-motivation and a flexible approach.
  • A positive attitude and be naturally caring and compassionate.
  • A commitment to high-quality care and to values and evidence-based practice.
