Key Responsibilities
  • Monitor and assess the physical health needs of service users, identifying areas for improvement and developing strategies to address them.
  • Develop and implement physical health interventions, using knowledge of current best evidence-based practice.
  • Collaborate with primary health services, developing and maintaining links with GP practices and other physical health departments.
  • Work closely with the multidisciplinary team to ensure seamless care and support for service users.
  • Monitor and maintain physical health dashboards, tracking progress and identifying areas for improvement.
Requirements
  • A relevant nursing qualification and registration.
  • Phlebotomy skills.
  • Strong team-working skills.
  • A high level of self-motivation and a flexible approach.
  • A positive attitude and a natural ability to care for and support others.
  • A commitment to high-quality care and to values and evidence-based practice.
  • Understanding of the physical health care needs of individuals with enduring mental health difficulties.
